Forms
The Office Visit Consent for Minors form will allow you to designate another adult to bring your child in for care. A parent (or legal guardian) may complete The Consent for Minors Form to allow another responsible adult (i.e. a grandmother or babysitter) to bring the child in for their medical care.

At Visalia Medical Clinic, we do whatever we can to accommodate our patients and their medical needs. That includes contracting with insurance companies to ensure payment of medical services.
Insurance
Medicare and several other insurance plans are filed automatically by the clinic when the proper information is provided. The patient is expected to pay all co-payments and services not covered by insurance at the time of service. As a courtesy to our patients, we file a claim if the coverage is through one of our non-contracted insurance companies. Our Patient Representatives are available to assist with billing and insurance questions from 8 a.m. to 5 p.m., Monday through Fridayat 559-741-2160.

Car/Home Injuries
An injury resulting from an accident (auto, home, etc.) is usually covered by insurance other than your health care carrier and should be billed to that liability carrier. We will be happy to assist you with that billing. You will also be required to make a minimum payment of $100 each time you come in for services. The clinic does not accept a lien for charges connected with an accident.
A statement of your account will be prepared and mailed to you on a monthly basis, when there is a patient responsibility portion due.
