For many of our patients, being able to recover from a surgery or illness at home with friends and family is an important part of the healing process. That is why Adventist Health Home Care Services offers a skilled, in-home health care team committed to the safety, comfort and independence of our patients. The Home Care team works closely with physicians to provide exceptional care that is personalized for each patient; nurses are available 24 hours a day, seven days a week for scheduled or on-call visitations.

Our Services

  • Personal care: Assistance with activities of daily living, such as personal care, bathing and dressing
  • Administration of antibiotics: Assistance, education and support of how to administer and mange IV infusion in the home setting for appropriate self-care
  • Nutrition: Education on proper nutrition, administration of feedings and aspiration prevention
  • Geriatric case management: Education and self-care techniques for managing acute and/or chronic conditions, when to seek medical attention, and when and how to properly take medications
  • Home diabetic programs: Education on proper diet, medication compliance and checking/monitoring of sugar levels in blood. Assistance in choosing user-friendly equipment, such as a glucose meter
  • Hydration: Education on monitoring hydration, detecting early signs of dehydration and preventing infections.
  • IV home therapy: Lessons in administering/managing IV infusion and preventing infections
  • Ostomy & wound care: Patient/caregiver education on how to care for stoma or wound, as well as infection prevention, how to dress a wound, and other self-care techniques
  • Rehabilitation: Education and training on gait, muscle strengthening and home exercise. A home safety evaluation may be conducted and recommendations are made to a patient’s doctor about any measures and/or equipment that may be needed. An occupational therapist can work on basic motor functions and assist in activities of daily living
  • Social services: Help in the assessment of social and emotional needs, counseling, financial and community resources
